Therapeutic Care Support Workers

We are offering an exciting opportunity to join the UHP Hospital to Home team .This service is being developed to support early discharge from hospital and provide therapy reablement in the community to frail and vulnerable Adults across Plymouth.

The Hospital to Home team will be an integrated service working in partnership with our health and social care colleagues to help reduce the need for people to remain in hospital and to help to re-build independence in their own home.

We are looking for Therapy Support Workers who will be supporting timely discharge by working alongside therapy colleagues and patients in the acute hospital setting to define and agree therapeutic plans in order to achieve their identified goals at home.

On discharge, the therapeutic care support workers will be providing therapeutic interventions and exercise to service users to maximize their potential and achieve independence to remain living in their own home. This will include intimate personal care, continence care, Moving and Handling to continuously work towards the service user's independence.

You will possess a minimum of NVQ level 2 and have an appetite for personal development. You will possess enthusiasm, motivation and be passionate about promoting independence in frail and vulnerable people.

You will receive professional ongoing training, comprehensive induction and the full support of Managers, Occupational Therapists andTeam Leaders. Although the service will be Occupational Therapy led, you will have access to Physio, Speech and Language therapy teaching to ensure you can deliver a fully holistic therapeutic package.

You will be expected to work on a rolling roster, including weekend shifts. Service hours are between 07:00-22:00, 365 days of the year.

You will need to travel across the area, and you will be paid travel from your office base.
